To ensure a smooth and efficient evaluation process at Mindful Vet Assessments, it’s important to come prepared. Here’s how you can get ready for your evaluation:

Complete Intake Paperwork

Prior to your evaluation, you will receive an intake packet that includes necessary forms and questionnaires. Completing these documents in advance allows us to focus more time on your evaluation.

Intake Questionnaire

The questionnaire will ask for detailed information about your medical history, current symptoms, and any previous diagnoses or treatments. This information helps tailor the evaluation to your specific needs.
Be as thorough and accurate as possible when filling out the questionnaire, as it provides essential context for the assessment.

Gather Necessary Documentation

Collect all relevant medical records, including past evaluations, treatment notes, and any documentation related to your service-connected condition.
Bring your military service records, as these are crucial for verifying service history and any related incidents.
